I have vaccinated family wanting to come in February. Do they need to first stay in a Bangkok “sha” hotel for a night ? Meaning I cannot pick them up my self at the airport?

February is three months away, so speculation.

Under current rules, 'no', you obviously can't pick them up yourself.

I am really struggling to understand why anyone would use the sandbox anymore since fully vaccinated people only have to stay one night in an hotel waiting for their test results done on arrival before being off to wherever they wish. In fact, even with the sandbox you have to stay in your room the first night waiting for the same test done on arrival. Very strange.

If they're not from one of the 63 approved countries they're not eligible for Test'n'Go, so if vaccinated the Sandbox is the next best choice.

It's for those from countries not on the Thai approved list at this point.
